Exemptions to school-required vaccines? JamPot requires all students to be vaccinated against certain diseases unless they have a Certificate of Medical Exemption on file at the school. Only Medical Exemptions will be allowed.
MEDICATION ADMINISTRATION
If your child is on a medication that will need to be administered at school, please use this link to download the medication administration permission form. Please note - all of our teachers are trained and certified in medication administration.
